If you have access to the Issue Management app, you can create issues against a plot or checklist item.
To add an issue against a plot:
Tap to open the plot.
Tap the black CREATE ISSUE button at the top of the section list.
To add an issue against an item:
Tap to open the section containing the item.
Tap to expand the item.
Tap the Add Issue button.
See Creating an Issue for further instructions on issue creation. If the issue was created against an item, you will now be able to see a link containing the issue number and subject when you expand the item. Tap the link to open the issue in Issue Management.
